This privacy policy sets out how our organisation [Sinéad Moore, trading as Present Health] collects and uses personal data in the operation of our business.

What data do we collect?

Present Health collects the following data through a combination of direct (i.e., client-practitioner consultation) and indirect (i.e., website, third parties) engagement, which is shared by digital or in-person means.

• Personal identification information which identifies you or can be used to contact you i.e., your name, address or e-mail address, phone number etc. This information is voluntarily provided to us by visitors to our various online channels and is used by us to respond to requests and submissions.

• Aggregated statistics from all visitors to Present Health’s website and digital channels. This information is anonymous and provides us with important insights on how visitors use and interact with our website. This helps us to ensure a positive user experience including the ongoing appeal of our marketing efforts. 

• Personal medical information which provides us with data strictly required to provide the product/service to the client. This information can include name, gender, date of birth, address, telephone numbers, email addresses, a person’s emergency contacts, health insurance numbers, and patient treatment notes and records. This can include sensitive information such as health records.

How do we collect your data?

We directly collect Personal Information if you send an enquiry to us from our website or social media, use Cliniko, or get in touch with us by chat, email, or phone. We also collect Personal Information if you engage with us directly, for example, client consultations or events.

Present Health may also receive your data indirectly from the following sources:

• When you engage with Present Health via Cliniko - a practice management system for clinics and allied health practitioners, which is fully GDPR compliant. You can view the steps Cliniko take to be GDPR compliant, here. This may include sensitive information such as personal details and health records, specified above.

• Browser and hardware data, such as IP address, type of device, operating system, browser type, screen resolution, language, device make and model, as well as the versions of the above mentioned services.

• Cookie and tracking technology data, which would include pages visited, time spent on pages, language preferences, and other anonymous traffic data.

• Transaction and payment processing data, when you pay online via Stripe or SumUp..

How will we use your data?

Present Health collects your data so that we can operate the business and provide the service to users. The personal information data we collect is used to:

• Provide you with bespoke services and products, namely treatment plans and natural medicine prescriptions related to your particular problem/condition.      

• Create and maintain individual profiles for delivering all-round customer service, billing, identification, authentication, and for contacting you if we need to.

• Get in touch with you about special offers on other products and services we think you might like, as well as news and tips. You have the option to unsubscribe at any time.

• Analyse trends of Present Health digital channels, monitor usage or traffic patterns (including to track users’ movements around the website) and gather demographic information about our users.

Present Health will never share your data with third party companies so that they may offer you their products and services, and will never share or distribute personally identifiable information without your consent. However, we may share your information with selected third parties such as:

• Other health care professionals and sub-contractors as part of your treatment plan, but only if we have obtained your prior consent.

• As part of anonymous and aggregated data for analytics and search engine providers that assist us in the improvement and optimisation of our site.

When personal information is transferred to outside service providers, it is done under a contract which requires the companies to use the shared information only for the limited purposes for which it is provided to them, and to agree to hold the information securely and confidentially in accordance with this privacy policy, and applicable law.

How do we store your data?

Encrypted data is stored in a secure online database and personal information in hard copy is stored in a locked filing cabinet. Access is restricted to those who have a need to know.  We store the personal information you provide in order to provide you with the information, products, and/or services you request and may be used to provide you with additional information about our services we believe may be of interest to you. 

Present Health will keep personal data for no more than 10 years from the termination of the relationship for the purposes of service and obligations; and for a maximum period of 24 months where the processing is carried out solely for marketing purposes; or as requested by you. Once this time period has expired, we will delete your data by removing it from our systems and securely destroying hard copy information held. 

In the event that there is a breach and your Personal Information that we have collected directly is at risk, you will be notified within 72 hours of discovering the breach. You will be informed of what information is at risk, steps that we have taken to ensure your safety, and what action we are taking or have taken to rectify the breach.

What are your data protection rights?

Present Health would like to make sure you are fully aware of all of your data protection rights. Every user is entitled to the following:

• The right to access - You have the right to request Present Health for copies of your personal data. We may charge you a small fee for this service.

• The right to rectification - You have the right to request that Present Health correct any information you believe is inaccurate. You also have the right to request Present Health to complete information you believe is incomplete.

• The right to erasure — You have the right to request that Present Health erase your personal data, under certain conditions.

• The right to restrict processing - You have the right to request that Present Health restrict the processing of your personal data, under certain conditions.

• The right to object to processing - You have the right to object to Present Health’s processing of your personal data, under certain conditions.

• The right to data portability - You have the right to request that Present Health transfer the data that we have collected to another organisation, or directly to you, under certain conditions.
